Intructions for DNA Processing of kits recieved at the World Dog Show 2011

Step 1:	Collect a DNA Sample according to the instructions in your kit.
Step 2:	Place the DN Numbered Barcode Sticker on the swab sleeve.
Step 3:	Write the dogs name on your swab sleeve.
Step 4:	Use the Form below to provide Customer and Pet information.
Step 5:	Mail your samples to Alvalley LLC, 1836 E. Amber Lane Apt. 202, Urbana, IL 61802 - U.S.A.
Step 6:	When your samples arrive at the lab, you will receive an e-mail with your DNA world Pet Registry login information. In approximately 10 days the DNA samples will be processed and will be ready for Parentage Certification.
